Where to chop...

We have just moved into a new house and inherited an orchard with a good mix of plum, pear and apple trees. I'm fairly confident with day to day pruning and am in the middle of tidying up the rest of the trees, however there is one particular young but sickly looking pear that has got me rather stumped...
What seems to be the main trunk is growing at a 45 degree angle, with some offshoots then growing vertically. I'm thinking long term how to get this into a place where the tree can grow successfully, but am not sure what to do. I'd thought about cutting off the 45 degree trunk and betting on one of the offshoots growing vertically becoming the main trunk, or instead cutting off the offshoots and trying to bring the 45 degree trunk up so that it becomes vertical over time, but frankly I'm not sure either way.
If anyone can take a look at the photo and give their thoughts on how to best tackle this I'd greatly appreciate it.
