Help! Hedge species? Disease?

Hi there,
I'm pretty new to all this. We have a hedge out the front of our house and it's in an awful state! Can anyone identify the species please? I understand it's not laurel, privet or box. But I have no other ideas?
Perhaps more importantly, can anyone identify the disease? I'm wondering if it's powdery mildew.
With a 'diagnosis' I can Google a treatment... but if you want to offer up suggestions for that too that would be great!

It's not something I've had to deal with so not sure of the best treatment
Wisdom is not putting it in a fruit salad.
See link for options. I'd cut the whole shrub right down and rake off the top two inches of soil and lay new mulch down.

It looks like I've missed the time of year to attack the larvae but at least I'll know for next year!
