Help with my Apple Tree please

Hey there. I'm new to the forum. My apple tree has been in over 15 years, about 3 years ago it stopped flowering as much and seemed to have some sort of disease. The apples it does produce are scabby. I cut it back last year and put a vitax winter tree wash on hoping that would fix the issue but again there are limited flowers this year and appear to be same issues. Please someone tell me what disease it has and what I should treat it with. It has some sort of fungus on the branches this year too.
