I am wondering what the attached “weed” is in my grass. I have tried weed killer to no effect, so I would be grateful for further advice and what exactly is it? Thanks in advance.

Looks to me like Mind-Your-Own-Business which is really difficult to control in lawns as it is resistant to the uesual lawn treatments.   However, September is a good time to dig it up by hand, refresh the soil levels and sow new grass.
