Hebe ‘True Love’

This plant has collapsed and appears to be dying. Can anyone please advise on what may be the cause and, if possible, what can be done to revive it.

    Dryness? Planting this time of year if it's been dry is always risky, plants will need a lot of watering to survive. 

    Or, vine weevil? Dig it up and check the roots for grubs and/or damage.

    If it could be revived, you will need to re-dig the area to make the soil more manageable. Plenty of compost so the roots can start to settle into. I suspect your shrub is delicate, and the soil is too heavy and now dried up in the hot weather. Not ideal conditions for a shrub that's probably grown in controlled environment.

    Once you have done that, water around the base every 3-4 days in the warm weather. A layer of bark chip will help keep the base less damp so less likely for low lying branches to rot.
