Apple tree has fallen over - advice?

Hello All,
I'd appreciate some advice - we have an apple tree which produces delicious apples. It has apparently been in the garden for 15 years+ although it is only c. 5 foot tall. Unfortunately it has fallen to one side, since when it keeps getting lots of aphids, has curling leaves, and is only producing apples along one of the main branches.
What should I do? Is it worth digging the whole tree up and replanting it straight? Or would that risk damaging it or killing it completely? I've propped it until now but that doesn't really resolve the situation.
