Had a problem years ago with this in a large garden I used to help maintain .
The 'boot-laces' (rhizomorphs) seemed to go on forever from the primarily infected plant .
Lost many Malus , Betula , Solanums , Berberis plus numerous others in an incredibly short time . Dramatic and unusually heavy flowering preceded death .Not at all nice when you cherish the plants involved
I don't know if it can be 'starved' like you say ; attacking mainly woody plants it has also been known to infect herbaceous material too .
In this manner I would have thought it would be fairly limitless in its infectious capabilities !
A copper-based fungicide (Armillatox) used to be moderately effective ; think this was banned years ago by our friends in the EU !
