Help me identify plant which is taking over garden

Can anyone help me identify the plant below. Earlier in the year I dug out the roots of what was there, put sheeting down and covered it with a good thickness of bark, but still it comes through.
It's also now coming through our lawn and patio.
What can I do to control it? Would a spot weed killer help?
